Unlike in the past when the GP practice was your first port of call for minor medical problems, community healthcare is now delivered by a wide range of health professionals working at different sites across the local area.  We have therefore put together a signposting document for you to refer to so that you can easily identify and access the service you need without any unnecessary delay.
