Urinalysis Form
Sometimes patients are asked to hand urine samples into the practice to be tested in-house - you will be provided with a small brown bottle to collect your sample in. This may be for a chronic disease annual review or may be at the request of a GP or Advanced Nurse Practitioner.
In-house samples are tested by either the Treatment Room Nurse, Practice Nurse or a Healthcare Assistant. They require as much information as possible in order to take the most appropriate action on the results of the test. We therefore ask that patients aged 16 and over complete the Urine Testing Proforma and hand this in with their sample of urine.
